Creatinine-based equations should be used with caution in patients with very low muscle mass (like cachectic or cirrhotic patients). These patients will have a much lower creatinine excretion rate than predicted by the equations above.
Further, this formula does not correct for race. It is known that African Americans have a higher muscle mass than Caucasians; hence, African Americans will have a higher serum creatinine level at any level of creatinine clearance.
